When you request a consultation, we may collect your name, phone number, email address, care preferences, preferred contact method, and any information you voluntarily include in your message. Online client intake is temporarily unavailable. Please call us to arrange an appropriate secure intake process.
We use this information only to respond to your inquiry, evaluate an application, coordinate care, discuss or arrange services, and provide requested follow-up. We do not sell your personal information.
Consultation requests are processed by FormSubmit. Online client applications and attachments are not accepted while secure intake is unavailable. Application information is not placed in page URLs or intentionally stored in the applicant's browser.
Do not use the consultation message field to send medical records, Social Security numbers, payment details, or other highly sensitive information. Do not email medical records or identity documents. Call us to arrange a secure intake.
